The art of medicine includes a medical history, a physical examination, a reasoned assessment, diagnostic testing, and a plan—often referred to as SOAP (subjective, objective, assessment, plan). The medical history is often limited to the patient’s medical history, and the heath history of genetic relatives. The medical history must be expanded to include the health of all members in the household, including those not genetically related; pets and animals in the household; and the medical history of other close contacts, to the extent known, including intimate contacts, caregiver contact with a person with a chronic disease, and animal contact outside the household. A medical history that includes only genetic relatives overlooks important information that could direct the provider to the cause of the presenting symptoms and presenting chronic disease. An expanded medical history could also direct medicine to new discoveries in chronic disease.
